HNR 101

I constantly hear people complain about things medical staff did or din't do. I always reply, "Do you have documentation?". Do you have proof that what you are saying is fact. The answers is frequently, "well I said to them......, and they said to me......" He said, she said does not win a grievance or an appeal or get you what you want. There are forms used to request to be seen by medical. Let's call these "Health Needs Requests" or "HNRs". Preferably your HNRs are in triplicate.

Here is a basic senario:

HNR #1: "I have an open lesion and need to be seen"
Response: "Referred to HCP (health care provider)"

HNR#2: "On January 1 I told you in and HNR that I have and open lesion. You responded "referred to HCP". It has been 2 weeks and I still haven't seen a doctor. When will I be seen for the open lesion that I have."
Response: "You are scheduled for the 18th"

Keep your copies with their responses. That's documentation. Do not over load medical with HNRs. Pick your battles. When its important, do it right. Document everything.
